Our Financial Policy
Thank you for choosing our office for your dental needs. Dental treatment is an excellent investment in your medical and psychological well being. Financial considerations should not be an obstacle to obtaining this important, life enhancing care. Frontier Dental Arts is flexible in regards to your finance arrangements, however, we are a "Fee for Service Dental Provider." Our staff is always available to answer your questions and/or assist you in any way we can.
For our patients with dental insurance: We are happy to assist you with the necessary forms to help you receive the full benefits of your coverage. The insurance relationship constitutes an agreement between the carrier and the patient. As such, we can make no guarantee of estimated coverage or payment. However, please know that we will do everything possible to see that you receive the full benefits of your policy.
Many insurance companies do not pay 100% of the claim; you may be asked to pay your estimated amount for treatment at the time of services. Because we cannot guarantee your exact insurance coverage; there may be a balance remaining after your insurance payment is received. You will receive a statement of the balance, which is due upon receipt of the statement.
NOTE: If your insurance does not cover the services rendered, you will be responsible for payment at the time of services.
Payment for Services
Fees less than $300 are due at the time service. We accept cash, personal checks, or credit cards (MC, Visa, Discover and American Express). For fees larger than $300 we can discuss some payment options that may be available to you. There is no guarantee of credit being extended to anyone. We do accept Care Credit (Apply today) .
Payment Options
Frontier Dental Arts is happy to offer a 5% prepayment courtesy for all treatment over $750 that is paid in full with cash or check prior to treatment commencing.
Fees of $300 to $1000 – Minimum of 50% down with the balance paid in full within 30 days.
Fees of $1000 to $2500 – Minimum of 50% down with the balance paid in 3 equal monthly payments.
Fees of $2500 and over – Minimum of 50% down with the balance paid in 6 equal monthly payments.
You may choose to make payments ahead of the time of services as a kind of “Lay Away Plan”. Treatment will commence after the estimated patient portion has been paid.
Orthodontic treatment fees - Minimum of 30% down with the balance to be paid over the duration of the estimated time of services in equal monthly payments.
NOTE: The account balance must be paid in full prior to removal of any appliances.
Invisalign treatment fees – Minimum of 50% down with the balance paid in 3 equal monthly payments.
Other financial payment options may be available, please ask for information on these products.
You can pay for your office visits and any treatments by cash, check, credit or debit card at the time of treatment. We also accept most major dental insurances.
